On September 11, 2025, Licensing Program Analyst (LPA) Grace Donato conducted an unannounced Case Management-Other visit. LPA met with Program Director, Renz Corrales and explained the purpose of the visit.
LPA delivered an immediate exclusion letter for two staff members who are currently working in the facility. One staff member (S1) was present at the facility and was escorted out by the Program Director.
The letter for S1 was also served in person by the LPA.
The letters was given to and reviewed by the Program Director..
This report is reviewed and discussed, and a copy is provided.
