Facility Evaluation Report
On August 28, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management visit to follow up on an unusual incident. LPA met with Program Director, Renz Corrales and explained the purpose of the visit.
On 8/18/2025, the facility reported that on 8/15/2025, at 10:30 AM during the community outing and while staff members were assisting and supervising the other residents in the park, resident #1 (R1) and resident #2 (R2) were in the van by themselves and R1 who was sitting in the back of the van approached R2 who was sitting in the passenger seat and grabbed his/her left leg and started masturbating.
On 8/16/2025, the facility was informed by R2's residence that R2's left knee was swollen and bruised.
On 8/18/2025, the facility was informed that R2 sustained an injury to the left knee.
On 8/19/2025, R2 was seen by a physician and the discharge diagnosis was Left Fibular Fracture.
During today’s visit, LPA observed R1, interviewed the program director, facility staff and collected documents and requested training records to be submitted to CCL by 9/2/2025.
No deficiency is cited today. This incident requires further investigation.
This report is reviewed and discussed with the program director and a copy is provided.
