Facility Evaluation Report
On June 5, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management visit to follow- up on an incident that was reported by the facility. LPA met with the administrator and explained the purpose of the visit.
On May 27, 2025, the facility reported to CCL that on May 26, 2025, at 3:05PM, resident #1 (R1) who resides in Memory Care Unit was brought to the first floor for a celebration and at 4:03PM, caregiver went to assist another resident and at 4:10PM, R1 was seen by the Engagement Director outside of the facility and R1 was escorted back to the facility.
During today’s visit, LPA observed R1, interviewed the administrator and requested for documents to be submitted by 6/6/2025.
This report is reviewed and discussed with the administrator.
