Facility Evaluation Report

On April 14, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced case management- facility closure visit. LPA met with program manager, Jaime Ortiz and LPA explained the purpose of today's visit.

Licensee notified CCLD of facility closure on 3/24/2026 as the licensee is no longer interested in maintaining a license.

LPA toured the activity rooms, nurse room, bathrooms, conference room, and kitchen and LPA did not observe any residents. LPA observed the facility was empty and almost all the furniture was moved out.

CCLD will be proceeding with the closure. A forfeiture letter will be sent to licensee and the facility number 419210105 shall be closed.

This report is reviewed, and discussed with the program manager.
