Facility Evaluation Report

On 1/14/2005, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management- - Legal/Non-compliance visit. LPA met with the administrator and explained the purpose of today's visit.

LPA toured the facility and observed facility to be cleaned and tidy. The doors alarm observed to be working properly. The indoor and outdoor passageways were free of obstructions. The medication, sharps and toxins were observed to be locked and inaccessible to clients. 2-day perishable and 7-day non-perishable was observed. A comfortable temperature is maintained and lighting is sufficient for comfort.

During today's visit, there are no clients.

No deficiencies observed during this visit. LPA reviewed report with the administrator and a copy is provided.
