On March 25, 2026, Licensing Program Analyst(LPA) Murial Han arrived at the facility to follow up on a Decision and Order(exclusion) of staff #1 (S1) and staff #2 (S2). LPA met with administrator and explained the purpose of the visit.
According to the Administrator, S1 and S2 are no longer working at the facility and the administrator acknowledged that they have received the Decision and Order.
This report is reviewed and discussed with the administrator.
