On 11/13/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met administrator, Brian Raimundo and explained the purpose of today's visit.
LPA delivered an immediate exclusion letter for staff #1 (S1) and staff #2 (S2) who are currently associated in the facility. The administrator confirmed that S1 and S2 are no longer working at the facility.
The exclusion letters were provided to the administrator.
