Facility Evaluation Report
On 10/15/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced case management visit to follow up on an incident that was reported by the facility. Upon entry, LPA was informed by the receptionist that the administrator is no longer working at the facility and LPA met with the Sales Director and LPA explained the purpose of today's visit.
On 10/9/2025, the facility reported that staff #1 (S1) witnessed staff #2 (S2) and staff #3 (S3) holding resident #1 (R1)'s arm in bed and forcing R1 to take medicine.
During today's visit, LPA toured the Memory Care Unit, interviewed R1, and staff members.
LPA requested for documents to be submitted to CCL by 10/17/2025: R1's LIC 602, appraisal service needs and plan, and documents related to the incident.
In addition, LPA requested the Licensee to submit documents for an administrator to CCL by 11/6/2025.
This report is reviewed and discussed with the Sales Director and a copy is provided.
