On June 4, 2025, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced case-management visit. LPA met with Director of Health Services, Amyda Astrero and explained the purpose of the visit.
The purpose of today's visit is to deliver an immediate exclusion letter to exclude an Staff 1 (S1) and Staff 2 (S2) from the facility.
The letter was given to the Director of Health Services.
This report is reviewed and discussed with the Director of Health Services and a copy is provided.
