On September 29, 2023, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced case management visit. LPA met with Administrator, Robert Snee, Director of Health of Services, Amyda Astrero and Assistant Director of Health Services, Hazel Yabut and explained the purpose of the visit.
The purpose of today's visit is to deliver an immediate exclusion letter to exclude an employee of the facility.
The letter was given to the Administrator.
This report is reviewed and discussed with the Licensee and a copy is provided.
