On February 3, 2023, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced visit and met with the Assistant Director of Health Services, Hazel Yabut and explained the purpose of the visit.
The purpose of today's visit is to deliver an immediate exclusion letter to exclude an employee of the facility.
The letter was given to the Assistant Director of Health Services.
This report is reviewed and discussed with Assistant Director of Health Services. A copy is provided.
