On November 13, 2025, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case-management visit. LPA met with Administrator, Jim Sidoti and explained the purpose of the visit.
The purpose of today's visit is to deliver an immediate exclusion letter to exclude a Staff 1 (S1) from the facility.
The letter was given to the Administrator. This report is reviewed and discussed with the Administrator and a copy is provided.
