Facility Evaluation Report
On April 10, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Annual Inspection and met with the Administrator, Nikita Desai. The facility file review was conducted at the Regional Office and additional records were requested and reviewed on site. The facility is licensed for 15 Elderly Adult residents and is currently operating at the capacity of 14 Elderly Adult residents. For a 740 facility type. The facility is cleared for 8 of those 15 to receive Hospices services but currently there are no residents on Hospice at this time.
LPA Mixson toured the facility along with the Administrator, Nikita Desai, and made observations pertaining to the Required Annual visit. LPA inspected the facility inside and outside. There were no obstructions or debris to the indoor or outdoor passageways observed. Additionally, there were no bodies of water seen on the premises at the time of this visit. The facility is a single-story facility located at 222 Washington Street Vista Ca.92084
Physical Plant: The facility phone number is (760) 936-8887, and it is operable. LPA Mixson observed the residents’ bedrooms, from number (1) through (9), and each was furnished as per Regulations and Title 22. LPA Mixson inspected the six facility bathrooms, and each was clean and had working utilities. The hot water temperature tested within regulations, and was logged. The bathrooms were clean, and appliances were operating appropriately currently. The facility is equipped with operating smoke detectors, 9 carbon monoxide alarms located in the bedrooms, and (2) carbon monoxide alarms are located in the hallway. There are two fire extinguishers. LPA Mixson observed required postings such as "If you See Something, Say Something,” the "Personal Rights," and the LTCO poster.
