The inspector’s account

Licensing Program Analyst, (LPA) Janira Arreola made an announced visit to the facility for the purpose of conducting a follow up pre-licensing inspection. LPA met with Administrator, Michelle Fuhrman , who was informed on the purpose of the visit. LPA conducted a tour, record review and interviews during the time of the visit. There are currently residents in care.

The applicant has updated their application and has been approved by the local fire jurisdiction for (42) non ambulatory and (6) bedridden residents in the main building. LPA reviewed the facility sketch and observed the building and rooms. LPA observed, emergency exits, fire extinguishers, and smoke and carbon monoxide alarms meet the department requirements. LPA observed rooms that would be designated for bedridden residents with second exit doors. LPA observed the exit route around the building leading to the parking lot.

On 12/16/2025 the second building at the facility was under construction and had not been yet approved by the local fire jurisdiction. LPA conducted an inspection of the second building on today’s date and observed construction is not yet completed. The administrator informed that the second building is currently being renovated to be attached the main building.

The administrator attempted to contact the applicant during the time of the visit, but was unable to. The administrator was informed that at this time the main building is approved, and the attachment is not approved by the department to house residents at this time. Additional follow up may be needed in order to move forward with the licensing process if the additional of the second building would like to be included. An exit interview was conducted where this report was reviewed and provided.
