Allegation: Facility staff physically abuses and beats residents

The detail of the complaint alleges R1 was beaten by staff, and that her roommates can corroborate the allegation.

At the time of the follow-up investigation, no relevant records were available for review.

On 7/24/23, LPA George conducted a health and safety inspection in response to the allegation. No health or safety concerns were identified during this visit. An interview was also held with the Administrator, who informed LPA George that the resident’s roommate—previously believed to be able to corroborate the allegation—had already passed away nearly a month prior to receipt of the complaint.

There is insufficient evidence to support the allegation mentioned above; Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
