The inspector’s account

On July 08, 2026, Licensing Program Analyst (LPA), Venus Mixson conducted an unannounced Required Annual and met with the Administrator, Susan Beals. LPA made introductions and stated the purpose of the visit.

LPA conducted a tour of the interior and exterior, there were no obstructions to the doorways at the time of this visit. LPA observed the Physical plant, floors, windows, and doors were clean, and fixtures and furniture were in good condition currently. The outdoor area was observed to be free of hazards and contained a shaded seated area for the residents in care. The sharp and dangerous objects were observed to be locked and inaccessible. The smoke detectors and carbon monoxide were operational. The facility has a thankless water heater that is set at 115 and can not go higher. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA observed facility kitchen had the ability to prepare food in clean environment and all equipment was operable. LPA observed the facility met the required two-day supply of perishable and seven-day supply of non-perishable food items. The staff and resident files were reviewed had the required documents at the time of this visit. The P&I for residents who received it was reviewed. Medication records were reviewed and found to be centrally stored and locked in a cabinet located in the kitchen at the time of this visit. Facility has an emergency and disaster plan, along with the infection control plan, and they met regulations at the time of this visit. LPA observed emergency supplies and first aid kit with all required items. Facility staff conducts quarterly disaster drills that meets Department requirements. The facility does not contain any bodies of water on the property. There are no firearms or ammunition stored at the facility. No deficiencies were cited at the time of the visit. An exit interview was conducted, and a copy of this report was discussed and given to Administrator, Susan Beals
