Facility Evaluation Report
On 07/18/25 Licensing Program Analyst (LPA) Javina George conducted an unannounced 1 year required visit. LPA was granted entry and met with Licensee/Administrator, Susan Beals, who was informed of the purpose of the visit. At the time of the visit there was (3) staff and (1) clients present. The rest of the clients were at the day program. The facility is licensed to serve six (6) ambulatory clients age range 18 to 59. LPA verified the facility contact information, and was found to be accurate.
The facility is a single story structure consisting of (7) bedrooms and (3) bathrooms, (1) room and (1) bathroom is designated for staff use. The is a kitchen, dining area, common area, with a backyard with shed that is being used for storage grapevines and garage. The facility does not have any bodies of water or firearms on the premises. The facility was observed to be clean and free and any obstructions in the passageways. The smoke and carbon monoxide detectors were tested and found to be operable.
LPA observed the facility to have an adequate food supply, as there was a 2-day supply of perishable and 7-day supply of non-perishable food supply. LPA reviewed (3) staff files. All staff present were observed to have criminal clearance and with valid CPR/First Aid Certification. The administrator Susan Beals is in possession of a valid administrator certificate that expires on 10/01/26. All client files were reviewed, and had the require paperwork. The personal and Incidental funds were reviewed with no discrepancies found.
All client medication are stored in metal cabinets located inside the garage, emergency supplies were present such as flashlights, portable toilet and emergency food. The facility is conducting the emergency disaster drills on a quarterly basis, with the last drill being conducted on 07/09/25. No deficiencies were issued during today's visit. An exit interview was conducted where a copy of this report was reviewed and provided to Susan Beals, Licensee/administrator.
