On April 29,2026, Licensing Program Analysts (LPAs), Venus Mixson and Mia Lankford arrived at the facility unannounced in order to conduct a facility closure visit. LPAs were not able to meet with anyone due to the facility being closed.
LPAs toured the outside of the facility and made observations.
There were no observable Title 22, Division 6, Regulation violations noted during the time of this visit. There were no residents or staff present at the time of the visit.
An exit interview was not able to be conducted due to facility closure. A copy of this report was mailed the Administrator, at the last known address.
