Facility Evaluation Report
On April 09, 2026, Licensing Program Analyst (LPA) Venus Mixson, Regional Manager, Reyna Lacey, and Licensing Program Manager (LPM) Jazmond Harris met with Leslie Wang in the office.
Leslie Wang reflects as the licensee of the facility. She reported she is not the Licensee and that she sold the business to Rolando Corpuz about four years ago. Leslie was unable to provide documentation of the sale nor proof that notifications were sent to the Department. Leslie reported that she will provide the documents by April 17, 2026.
Leslie was provided LPM Harris' contact information to submit documents.
An exit interview was conducted where this report was reviewed and provided to Leslie Wang.
