Facility Evaluation Report

On April 07, 2026, Licensing Program Analyst (LPA), Venus Mixson made an unannounced Case Management health and safety visit, and met with the Administrator, Rolando Corpuz. LPA explained the purpose of the visit.

LPA conducted a tour of the facility along with the Administrator, currently there are ten residents and three staff members. LPA confirmed that the facility has operable utilities (gas, electricity, water) and adequate food supply. There were no immediate health and safety concerns noted at the time of this visit. LPA reviewed 10 of 10 resident files.

LPA spoke with Administrator Rolando Corpuz to inquire about the closure letter that was issued to each resident and/or their Responsible party. Based on information obtained from the interviews as well as a copy of the letter issued, the requirements of Health and Safety (H&S) code 1569.682 were not met. There were various requirements missing including but not limited to, that the letter did not provide written notice no later than 60 days prior to the intended eviction. The letter was dated March 30, 2026, with an effective date of April 30, 2026. In addition, this facility has seven or more residents which requires that a closure plan be submitted to the Department and approved before the closure letter can be issued. A copy of the H&S code 1569.682 was reviewed and provided to the administrator. A deficiency will be cited. An exit interview was conducted where a copy of this report, LIC809D, H&S 1569.682 and appeal rights were reviewed and provided to the Administrator Rolando Corpuz. The administrator was advised to follow all of the requirements in that section and that if further assistance or clarification is needed, he can reach out to the Department for guidance.
