Facility Evaluation Report

On March 26, 2026, Licensing Program Analyst (LPA), Venus Mixson, and Regional Manager (RM), Reyna Lacey arrived unannounced for a Health and Safety case management visit and met with Administrator, Rolando Corpuz. CCLD staff introduced themselves and stated the purpose of the visit.

During the visit LPA Mixson and RM Lacey toured the facility inside and out and made observations pertaining to today’s Health and Safety inspection.

During the visit interviews, record reviews, and observations were conducted.

There were two staff present at the time of the visit, and 2 of 2 staff were not associated to the above listed facility. There will be citations for this Regulation violation. An additional staff who arrived later was associated to the facility, and contact was made with the Administrator who advised that this would receive immediate attention. The Administrator advised that a copy of the LIC 500 Facility staff Roster and staff schedule, along with documented proof of medication training was provided by close of business day tomorrow (03/27/2026).

Based on the information obtained from interviews, record reviews, and observation the facility will be cited for the above listed violations.

An exit interview was conducted. A copy of this report, along with the Appeals Rights, and Citation was reviewed and given to the Administrator, Rolando Corpuz.
