Complaint Investigation Report

Additional feedback provided during interviews with staff and residents revealed that caregivers make the residents’ bed and clean the residents’ room daily. Based on observation and interviews, there is no concerns that would prove that the facility is dirty or malodorous thus the allegations are UNSUBSTANTIATED.

An allegation(s) finding of unsubstantiated means, although the allegations may have happened or are valid, there is not a preponderance of the evidence strand to prove the alleged violations did or did not occur.

An exit interview was conducted, and a copy of this report was provided to Starsha Clark.
