Facility Evaluation Report
On 07/09/25 Licensing Program Analyst (LPA) Javina George conducted an unannounced annual required visit. LPA was granted entry and met with Administrator Evelyn Jackson, and informed her of the purpose of the visit. At the time of the visit there was one (3) staff and one (1) client present. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following:
The facility was observed to be clean and clutter free. There was ample linen, emergency supplies and food supply. The sharps, chemicals, and medications were observed to be locked and inaccessible to clients in care. The smoke detector and carbon monoxide were tested and found to be operable. The fire extinguishers were last checked 12/2045, and are stored inside a locked case. There are no guns or ammunition or bodies of water on the premises. Client Personal and Incidental funds were not reviewed as the funds were not at the facility. The facility is conducting emergency disaster drills on quarterly basis, with the last drill being conducted on 05/15/25.
A records review was conducted and all staff present were observed to have obtained criminal record clearance and to be associated to the facility. Staff were observed to have CPR/First Aid Certification. In addition the administrator Evelyn Jackson was observed to possess valid administrator certification, which expires on 06/18/27. The client files reviewed, and were observed for all required paperwork such as Admissions Agreement, Physician's Report, and current Individual Program Plan (IPP). During today's visit LPA gave a reminder regarding the facility annual fees that are due on or before 07/27/25 and provided the following PIN 832003, should the licensee wish to pay electronically. The governing body was observed to be in good standing. No deficiencies were cited during today's visit.
An exit interview was conducted where a copy of this report was provided to Administrator Evelyn Jackson.
