Facility Evaluation Report

On May 27, 2025, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Required Annual Inspection and met with Administrator , Elissa Ciacereeli. Facility file review was conducted in the Regional Office and additional forms were requested and reviewed on site. Facility is operating at 100% capacity of four Adults.

LPA Mixson toured the facility along with Administrator and inspected the facility inside and outside, and there were no obstructions or debris to the indoor or outdoor passageways currently. The facility is a single-story home located at 2808 Foothill Drive, Vista CA 92084.

Physical Plant: Facility phone number is (760) 598-1668 and it is operable. LPA observed Residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA inspected facility bathrooms, and the hot water temperature tested within regulations. Bathrooms were clean, and appliances were operating appropriately currently. Swimming pool had a locked and secure gate within regulation.

Facility is equipped with operating smoke detectors, carbon monoxide alarms, and Fire extinguishers, last services June 2024, and recharge date is scheduled for June 09, 2025. LPA Mixson observed required postings such as "If you See Something, Say Something," "Personal Rights" and the Ombudsman postings. Cleaning supplies and sharp items were kept locked and inaccessible to Residents in care. A designated storage space for residents and staff files was observed and it was locked and inaccessible to residents in care currently.
