Facility Evaluation Report
On April 22, 2026, Licensing Program Analyst (LPA) Venus Mixson arrived at the facility unannounced to conduct the Required Annual Visit and met with the Facility Manager, Emma Bailey. LPA introduced herself and stated the purpose of the visit. The facility file review was conducted in the Regional Office and additional forms were reviewed and requested on site. The facility is Licensed for four Adults Residents, for a 735, facility type.
LPA Mixson toured the facility along with the Facility Manager and inspected the facility inside and outside, and there were no obstructions to the indoor or outdoor passageways at the time of this visit. The facility is a two-story home located at 2248 Tierra Verde. Vista Ca.92084. With four bedrooms and three bathrooms, and an attached garage. Facility Manager informed LPA there were no pools or firearms are being kept at the facility. The clients served are adults between the ages of 18-59. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted staff interviews. LPA observed the following:
Physical Plant: LPA observed the client bedrooms. Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair. The outdoor area was observed to be free of hazards. LPA observed outdoor furniture for clients. Laundry equipment was observed to be in good working condition. The sharp and dangerous objects were observed to be unlocked in a kitchen drawer. No clients were present at the time of the visit, due to being at the Day Program.
