Facility Evaluation Report

Licensing Program Analyst (LPA), Natasha Persaud conducted a Case Management - Other visit. LPA met with Licensee, Sarai Marcelin and discussed the purpose of the visit.

Today, LPA briefly toured the facility and interviewed a client regarding an unrelated issue to the day program. No deficiencies were observed or issued. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Licensee, Sarai Marcelin whose signature below confirms receipt of these rights.
