Facility Evaluation Report
On March 20, 2026, Licensing Program Analyst (LPA), Venus Mixson made an unannounced Health and Safety visit and met with the Office Manager, Nicolasa Rojas. LPA introduced herself and stated the purpose for the visit.
On March 04, 2026, Community Care Licensing received information pertaining to an incident that occurred on 02/27/2026, at the AVID Day Program. Information was provided through the Special Incident Report and Shared Information, through the San Diego Regional Center. Additionally, the information was provided by the Licensee of the above listed agency.
Information obtained indicated that on the previous Friday 02/27/2026, at about 12:00pm, one of the residents was displaying challenges with behaviors that included pulling staff's hair. At that time the staff indicated that staff inappropriately restrained the resident. Additional information obtained from interview with the Licensee advised that staff did not follow the CPI intervention techniques and as a direct result of this violated the residents personal rights. This inappropriate response to residents behaviors, by the staff was a violation of the "No-Hands" on company policy. The Staff that did not follow the intervention techniques was separated from employment immediately at that time that the incident was brought to the attention of the management team. There were no observed Health and Safety concerns observed, or Regulation violations cited during today’s visit.
Based on information obtained during today’s visit there are no concerns at this time. An exit interview was conducted, and a copy of this report was reviewed and provided to the Office Manager, Nicolasa Rojas.
