This is an official report of an unannounced visit/investigation of a complaint received in our office on

06/10/2022 and conducted by Evaluator Stephanie Torres

COMPLAINT CONTROL NUMBER: 18-AS-20220610115555

Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility to initiate the investigation into the above allegation. The LPA met with Business Office Manager (BOM), Monica Flores, and informed her of the purpose of her visit.

The LPA conducted a staff interview, reviewed records and took copies of pertinent documentation. Regarding the allegation, "Facility did not issue a refund," it was alleged the facility did not provide a refund to Resident One (R1) after they moved out of the facility on April 16, 2022. Flores was interviewed and reported R1 was issued a refund and was processed on June 16, 2022. Email correspondence revealed R1's refund was started on April 27, 2022. Therefore, based on records, this allegation is deemed UNFOUNDED. A finding that the complaint is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis.

This report was reviewed with Flores and a copy was provided.
