Facility Evaluation Report
On June 03, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived unannounced at the facility and met with the Alma Carranza, Facility Manager. LPA informed the purpose of the visit was to conduct the annual inspection. The facility is licensed for four adult residents for a 735 facility type and is operating at 100 percent capacity, and within the scope of the license. LPA toured the facility along with Alma Carranza and made observations pertaining to the annual visit.
The facility is a two story home with six bedrooms and four bathrooms. Two bedrooms and one bathroom is designated for staff. Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair and were present. The outdoor area was observed to be free of hazards. The sharp items were observed to be locked and inaccessible to the residents in care. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility exceeded the required two-day supply of perishable and seven-day supply of non-perishable food items. The facility retains an emergency and disaster plan and emergency supplies. LPA Mixson reviewed documentation showing the facility's last fire drill was conducted this past Monday June 01, 2026, and was logged and documented. LPA observed the facility exits were clear from obstructions and debris at the time of this visit.
