Facility Evaluation Report
On 05/21/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and entry by Alma Carranza, House Lead where LPA explained the purpose of the visit. At the time of the visit there was (1) staff present, and (1) client present, the other (3) clients were on an outing. During today's visit LPA verified contact information, and will update accordingly. The facility annual fees that were due on or before 05/10/25, have not been paid, Licensee Isabel Sanchez the payment was resent via mail last week, again as the initial mail was returned.
The facility was observed to be clean and clutter free. There are several animals (goats, chickens, roosters, dogs, cats, and chinchillas). There is a shed stores garden equipment, and games, available for clients to use. The sharps, medications and other hazardous items were observed to be locked and inaccessible to clients. LPA observed the facility was observed to have the required 2-day supply of perishable and 7-day supply of non-perishable foods. The facility was observed to be in possession of emergency supplies. Emergency disaster drills are being conducted on a monthly basis, the last drill completed was on 05/01/25. The hot water was tested and measured to be within regulatory limits. The combined moke and carbon monoxide detectors were tested and observed to be operable. The fire extinguisher was observed to be fully charged with the tag in tact. LPA conducted a file review for all (4) clients and (3) staff. LPA reviewed the LIC500-Personnel Record and observed for all staff to have obtained criminal record clearance and to be associated to the facility. Staff were observed to possess valid CPR/First Aid Certification, and the Administrator Jennifer Woods to possess a valid Administrator certificate that expires on 04/26/27. The client files had the requires assessments and signed admissions agreements. Based on today's inspection, no deficiencies were cited at the time of the visit. An exit interview was conducted were this report was reviewed and provided to Alma Carranza, House Lead.
