The inspector’s account

On 09/18/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by House Manager Jennifer Powell, where LPA explained the purpose of the visit. At the time of the visit there were (2) staff and (3) clients present. Below are the observations made during today's visit. The annual fees have been paid, and the governing body is not in good standing. Per Powell an exemption form needed to be submitted, proof of completed form was provided during today's visit.

The facility was observed to be clean, clutter and odor free, the facility was observed to have the required postings. The rooms had the required furniture. The facility was in possession of extra linen and personal hygiene supplies. The medications, chemicals and sharps were observed to be locked and inaccessible to client in care. The food supply was sufficient as there was a 2 day supply of perishable and a 7 day supply of non perishable food items.

The emergency disaster drills are being conducted on a quarterly basis, the last drill was conducted on June 1, 2025, with a drill being due by the end of this month. The smoke and carbon monoxide detectors were tested and found to be operable. There are (3) fully charged fire extinguishers, that were last serviced on 05/17/25. The facility has a pellet gun that is stored inside a locked safe.

A file review was conducted of client and staff files. The client files were observed to have the required documentation. A review of client Personal and Incidental (P&I) funds were reviewed with no discrepancies found. Staff were observed to have obtained criminal record clearance and were observed to be associated to the facility. The staff files were found to have the required documentation, training, identification, and clearance forms. There were no citations issued during today's inspection.

An exit interview was conducted and a copy of this report was provided to House manager Jennifer Powell.
