Staff did not ensure the facility was free from pests

Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced visit to open a complaint investigation regarding the above-mentioned allegation. LPA identified herself and met with Monica McDade, Program Manager.

It was alleged that staff did not ensure the facility was free from pests. Interviews with staff revealed a staff observed a bug in the kitchen and advised the Program Manager about it. Interviews revealed that the manager called their pest control agency to come out and spray as soon as they were made aware of the issue. Interviews revealed that the pest control came out on 06/29/2026. The staff took items out of the cabinets and drawers in the kitchen and the rest of the facility was sprayed as well. Interviews revealed the pest control will come back and do a second round of spraying on 07/21/2026. Interviews with outside sources revealed that they come to the facilty monthly and that the last monthly visit for regular service was on 6/16/2026. Interviews revealed that they have been coming out to this facility monthly since 2010 for regular service.

The allegation of staff did not ensure the facility was free from pests is unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted with Monica McDade, Program Administrator. A copy of this report and Licensee Appeal rights (LIC 9058 03/22) were provided at the end of the visit.
