Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit to gather information regarding a client that went AWOL and returned. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Program Director Monica McDade.
The Department conducted a case management visit to speak with staff regarding an AWOL. Client #1 (C1), AWOL'd around 9:30 am on 02/12/2025 and left walking with staff shadowing them. Records show that C1 is not able to leave the facility unassisted. C1 has Epilepsy, Bipolar Disorder and Mild Intellectual Disability. Interviews revealed the client was about four houses down when the sheriffs arrived and spoke with C1 and put C1 in the car and brought them back after C1 asked for a ride in the police car. Interviews revealed that C1 made some comments about self harming/hurting them while at program and the program called the Administrator and explained the incident to them and asked that they come and get C1 from program. The same afternoon that staff picked C1 up from program they received a call from the program stating they were suspending C1 until they can have a meeting on how to best meet their needs after C1s comments. When C1 woke up the next day and started getting ready for program the staff had to explain to C1 that they were not able to go to program and that is when the behavior of crying started and when they walked off with staff following close behind. The client returned unharmed and staff ended up taking C1 to lunch and shopping to get their mind off not being at program. The client does not have a history of AWOLing. Based on the interviews, no deficiencies are cited today.
An exit interview was conducted with Monica McDade, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
