COMPLAINT CONTROL NUMBER: 08-AS-20250808154108
Licensing Program Analyst (LPA)Tiffany Holmes conducted an unannounced complaint visit to the facility to deliver findings on the above-mentioned allegation. LPA gained access to the facility, identified herself, and met with Evan Sitto, Administrator, to discuss the purpose of the visit.
LPA conducted investigation visit and was able to interview clients, facility staff, and outside sources. LPA also reviewed records, and conducted a physical inspection of the facility. It was alleged that staff hit a client. Interviews with staff revealed the staff denied hitting the client. Interviews revealed that Client 1 (C1) was not able to qualify for the interview. Interviews revealed that C1 and another client was walking around the lake with Staff 1 (S1). Interviews revealed while walking C2 was walking behind C1 and S1. Interviews revealed that C1 jumped on S1 back and attempted to get a piggy back ride. S1 asked C1 to get down so they could finish walking around the lake. They were feeding the ducks after this and S1 grabbed C1s ear muffs to tell C1 lets go and that is when C1 stated that S1 hit them. There are no interviews that revealed S1 hit C1. Interviews with outside sources revealed they have not had any complaints against the staff regarding hitting any clients. Interviews with another outside source revealed the staff are good with C1 and manage them very well. Based on the evidence obtained from interviews, and record review, the complaint allegation of staff hit client is unsubstantiated.
An exit interview was conducted with Evan Sitto, Administrator and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided at the conclusion of the visit.
