Complaint Investigation Report
Interviews revealed that the staff redirect the clients in situations that put the clients or themselves in harm. Interviews with clients revealed that the clients feel safe in the facility and that the staff are helpful and keep them safe. Interviews revealed when there are incidents that occur the staff step in quickly are right there to prevent the clients from engaging in a physical altercations. Interviews with an outside revealed the staff always try to prevent the clients from engaging in physical altercations and this time the staff were right there when it occurred and they immediately and removed the clients and had them in their own areas.
Based on the evidence obtained from interviews, and record review, the complaint allegation of the staff did not prevent clients from engaging in a physical altercation is found to be unsubstantiated; as there is not a preponderance of evidence to prove the alleged violations did or did not occur.
An exit interview was conducted with Edgar Casillas, Staff and a copy of this report along with Licensee/Appeal Rights (LIC 9058 01/16) was provided at the conclusion of the visit
