Complaint Investigation Report
It was alleged that the licensee did not arrange medical care for client and the facility did not allow client visitations. Interviews revealed that C1 did not miss any medical appointments. Interviews revealed that C1 was able to verbally express when they needed to see a doctor. Interviews with clients revealed they are taken to their medical appointments. Interviews with an outside source revealed that they did not have any issues with the facility taking any clients to the doctor.
Interviews also revealed that the clients are able to have visits at the facility. Interviews with clients revealed they have not been denied any visitations. Interviews revealed that during 2020 visits were scarce due to covid but in 2021 and after the visits would be in the facility and the visitors were able to sit inside or outside whichever was their preference. Interviews with an outside source also revealed that they have not received any complaints for this facility regarding visitations.
The Department has investigated the above-mentioned allegations and based on interviews, LPA observations, and records review, it was determined that the complaint allegations are Unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Ayad George via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
