Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management - Incident visit. LPA was welcomed by and identified herself to Tylecia Jacquess, Staff. LPA discussed the purpose of the visit.

Today's visit was (1) to get signatures for an amended report from 3/18/2025. The (2cd)is in response to LIC624 Incident Report, which licensee self-submitted to the CCLD San Diego Regional Office (received on 05/30/2025). According to the LIC624: Client #1 (C1) went AWOL (absent without leave) from the facility on 5/25/2025. [See LIC 811 Confidential Names List for a description of C1.] On 06/5/2025, client returned to the facility without any explanation of where they were or stayed.

As of today’s (06/10/2025) licensing visit, C1 has returned to the facility. LPA performed a facility tour and welfare check on the other remaining clients in care, finding no immediate safety concerns. LPA also reviewed pertinent records and interviewed relevant staff.

According to C1’s latest LIC602 Physician’s Report (dated 08/16/2024): C1’s primary diagnoses is Paranoid Schizophrenia. Their doctor determined that C1 was able to leave the facility unassisted.

According to the facility’s Absentee Notification Plan/Policy: After seventy two (72) hours staff are to notify law enforcement and the client’s responsible person and/or case manager.
