Facility staff extorted money from client.

Licensing Program Analyst (LPA)Tiffany Holmes conducted an unannounced complaint visit to the facility to deliver findings on the above-mentioned allegation. LPA gained access to the facility, identified herself, and met with Marvasia Smith to discuss the purpose of the visit. LPA’s visit consisted of delivering findings on the above-mentioned allegation.

LPA conducted a physical inspection of the facility, collected relevant records, and conducted interviews with clients, facility staff, and outside sources. It was alleged that facility staff extorted money from client. Interviews revealed on May.16, 2021 Staff 1 (S1) attempted to solicit money from Client 1 (C1) in exchange for pictures of their feet. Interviews revealed that S1 threatened C1 by stating they would have their significant other harm C1 if C1 didn't pay. Interviews revealed C1 did not pay S1 any money and C1 advised staff of the incident. Interviews revealed S1 was immediately terminated and did not have access to C1 any longer. Based on the evidence obtained from interviews, records review, the complaint allegation is unsubstantiated. An exit interview was conducted with Marvasia Smith and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided at the conclusion of the visit.
