Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit for the purpose of obtaining a signature and delivering an amended report. LPA was granted entry into facility by Oscar Lara, Administrator. LPA spoke with Administrator Oscar Lara and discussed purpose of the visit.

During today's visit LPA obtained a signature for amended LIC 9099 dated 07/29/2025 and Case Management LIC 809. LPA had to change the date on the LIC 9099 report from 2024 to 2025.

An exit interview was conducted with Oscar Lara, Administrator and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided at the end of the visit
