Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit. LPA was welcomed by and identified herself to Sally Saif, Administrator, LPA discussed the purpose of the visit.
Today's visit was to get signatures for an amended report from 04/23/2025. LPA needed to correct wording on prior 9099 and amended the report. LPA received signatures for the report on todays date.
No deficiencies were cited for the above incident. No deficiencies were observed or cited during today's visit.
An exit interview was conducted with Sally Saif, Administrator, to whom a copy of this report, the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
