The inspector’s account

Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit. LPA identified themselves to Sally Saif, Administrator.

Today's visit is in response to the AWOL of Client 1 (C1) date of incident: August 8, 2024. The facility properly reported C1 went AWOL from the hospital around 3 am Against Medical Advice (AMA). The facility's Missing Persons/Absentee Notification Plan was reviewed on this date with the Licensee. According to Licensee, the hospital did not notify the family nor the facility since the client is not conserved. Licensee contacted San Diego Regional Center (SDRC) after hours line, contacted C1s family/ responsible party (RP) and the Sheriffs Department.

LPA conducted interviews on this date. According to C1s latest physicians report dated 06/14/2024 it states the client cannot leave the facility unassisted. C1 has a history of AWOLing per their Individual Personal Plan (IPP) and the referral form 159 A from the San Diego Regional Center (SDRC). LPA received copies of pertinent information regarding C1.

Client returned to the facility on Saturday 08/10/2024 sunburned and unclean. C1 also appeared to be intoxicated and very tired. Interviews revealed C1 wanted to rest and they showered, shaved and ate dinner and laid down. C1 went to program on Monday Morning.

No deficiencies were cited or observed on this date.

An exit interview was conducted with the Administrator. A copy of this report along with Licensee/Appeal Rights (LIC9058 01/16) was provided to Saif at the conclusion of the visit.
