This is an official report of an unannounced visit/investigation of a complaint received in our office on
01/07/2025 and conducted by Evaluator Tiffany Holmes
COMPLAINT CONTROL NUMBER: 08-AS-20250107095916
Staff does not ensure resident is free from intimidation.
Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Monica McDade, Program Director
LPA previously conducted interviews with clients, staff, and outside sources, made observations, and obtained and reviewed pertinent records. LPA conducted the initial visit on January 15, 2025 and conducted a tour of the facility.
It was alleged that staff yells at client and staff does not ensure client is free from intimidation. Interviews revealed that Client 1 (C1) reported that staff were verbally aggressive and accusing C1 of doing things. Interviews revealed that C1 could not say who the multiple staff were that yelled at them, intimidated them or that was accusing therm of doing things. Interviews with staff revealed C1 was being disrespectful on January 7, 2025 and that C1 called the sheriffs department. The sheriff and Pert came to the facility, talked with both staff and clients. Interviews revealed that the staff did not tell C1 "that if they didn't like it there at the facility they could leave". Interviews revealed that staff denied the allegation of yelling at the clients and intimidating them. Interviews with staff revealed they meet the clients needs and respect the clients. Interviews with Program Director did not reveal any other complaints regarding staff and how they treat and talk to the clients.
The Department has investigated the above-mentioned allegations and based on interviews, LPA observations, and records review, it was determined that the complaint allegations are Unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Program Director Monica McDade via face time and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided via email. An electronic email read receipt confirms the documents were received.
