Complaint Investigation Report

Interviews revealed that S1 was not seen putting their hands around C1s throat at any time. S1 denied handling the resident in a rough manner. Interviews and documents observed revealed that C1 has a history of making up stories and not being truthful.

The allegation is unsubstantiated. An exit interview was conducted with Monica Mcdade, Program Administrator. A copy of this report and Licensee Appeal Rights (9058 01/16) were provided to Administrator after the conclusion of the visit.
