Facility Evaluation Report

Licensing Program Analysts (LPAs) Tiffany Holmes conducted an unannounced Case Management visit. LPA identified herself to Raafat Azeez, Staff we discussed the purpose of the visit.

Today's visit is in response to the AWOL of Client 1 (C1) date of incident: 03/23/2025. The facility properly reported C1 going AWOL. C1 returned on 03/23/2025 about 10 minutes after leaving.

C1 AWOL'd out of their room around 3am. Interviews revealed the staff heard C1s door open and did not see C1 in their bed. C1 had eloped through the exit to the backyard and was later found within 10 minutes around the facility. Interviews revealed that staff easily redirected C1 back to the facility because C1 stated they were "confused". Interviews revealed that C1 had a planned visit with their parent the day before.

Interviews and a record review revealed, C1 LIC 602 dated 10/21/2024 states they can leave the facility unassisted, and C1 does not have a history of AWOLing. The facility did not notify law enforcement due to the client being found so quickly. LPA conducted interviews on this date. LPA received copies of pertinent information regarding C1.

No deficiencies were cited or observed on this date. At the time of visit, C1 is at program.
