Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Murtadha Imran, staff.
Today's visit was in response to an LIC 624 Incident Report, which licensee self submitted to the CCLD San Diego Regional Office (received on 06/05/2026), involving Client #1 (C1).
During today’s visit, LPA performed a brief facility tour, observed copies of pertinent records, and interviewed relevant staff. According to the LIC 624 dated 01/27/2026 client is able to leave the facility unassisted. Client Awol'd on 05/31/2026 and returned on 06/05/2026. The facility reported that the client can leave without supervision and has a history of being AWOL for days at a time. Interviews revealed they have been in contact with SDRC and local law enforcement. The client was admitted to and discharged from a local hospital which reports that drugs were in their system.
No deficiencies were observed or cited during todays visit.
An exit interview was conducted with Murtadha Imran, staff to whom a copy of this report, the Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during at the conclusion of the visit.
