01/13/2026 and conducted by Evaluator Tiffany Holmes
COMPLAINT CONTROL NUMBER: 08-AS-20260113164502
Licensing Program Analyst (LPA)Tiffany Holmes conducted an unannounced complaint visit to the facility to open a complaint on the above-mentioned allegation. LPA gained access to the facility, identified herself, and met with Arlene Doulopoulos Administrator to discuss the purpose of the visit.
LPA conducted conducted a tour of the facility, and conducted interviewes. It was alleged that facility is not treating for rats. Interviews revealed that staff sprayed the facility and has a pest control coming to the facility to lay traps. Interviews revealed the facility constantnly has rats and they are in all three buildings. Interviews revealed that the staff has caught several but there are some still in building one where the kitchen is. Interviews revealed that the pest control came on 12/16/25 and observed high activity in bait stations for rodents. Interviews revealed on 12/22/25 the pest control came back to the facility and treated 3 rooms for bed bugs. On 01/12/26 the pest control came again and has an ongoing service for rodent bait systems. On 01/15/26, according to pest control documents, the pest control came out and serviced the facility by retreating the facility due to increased pest activity and reduced bed bug activity and a recommendation for the facility to steam clean the furniture. Interviews revealed the facility staff is treating the facility but the method is not working.
Based on the evidence obtained from interviews, the complaint allegation is substantiated. A substantiated finding means the allegation is valid because the preponderance of the evidence standard has been met.
A deficiency is cited per Title 22 California Code of Regulation on the 9099 D page. An exit interview was conducted with Arlene Doulopoulos Administrator and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided at the conclusion of the visit.
