Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management visit to discuss an incident that occurred at the facility on 02/08/2026. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Arlene Doulopoulos, Administrator.
On 02/11/2026, the CCLD San Diego RO received an LIC624 Incident Report which described Client 1 (C1) was using drugs in their room and over dosed with another client and their friend in the room. Interviews revealed emergency services (911) were called and arrived on the scene shortly after and administered Narcan to C1 and revived them.
The client went to the hospital on 02/08/2026 and returned about 4 hours after. After speaking with staff the interviews revealed that the staff did not see any drug paraphernalia but the sheriffs did in C1s room. Interviews revealed the staff do rounds to check on the clients activity and safety. Interviews revealed that they plan to increase staff monitoring of the rooms, provide more staff training on signs of drug usage and reinforce the house rules that possession of illicit drugs is prohibited.
No deficiencies were issued during today's Case Management visit.
An exit interview was conducted with Arlene Doulopoulos Administrator, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
