Facility Evaluation Report

Licensing Program Manager (LPM) Simon Jacob and Licensing Program Analyst (LPA) Tiffany Holmes conducted an Office Meeting with the Licensee, Margaux Merziotis. LPM explained the purpose of the meeting.

On today's date, the LPM, LPA and Mrs Merziotis met to discuss the licensee's compliance history, facility issues, fingerprinting issues and physical plant. LPM provided County resources to the Licensee and the facility is already linked to the Technical Assistance Program (TSP) and upcoming visit is scheduled for April 10, 2026. The Licensee will coordinate with the TSP to be in attendance at the next facility meeting. Licensee is also in the process of re-evaluating the current management and employee structure and will keep the RP informed of anticipated changes.

An exit interview was conducted. A copy of this report, along with Licensee/Appeal Rights, were provided to Licensee, Margaux Merziotis at the conclusion of the visit. Her signature on this form acknowledges receipt of these rights.
