Facility Evaluation Report
Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Melody Laflam, Caregiver. Charity Bailey, Administrator arrived during the visit.
Today's visit was in response to an LIC 624 Incident Report, which licensee self submitted to the CCLD San Diego Regional Office (received on 04/24/2026), involving Client #1 (C1).
During today’s visit, LPA performed a brief facility tour, collected copies of pertinent records, and interviewed relevant staff. According to the LIC 624 PERT was called on 04/16/2026 and took C1 to API inpatient for verbal threats to kill everyone at the facility and increased aggression, along with being under the influence of illegal substances. It was noted that the an outside source observed C1 having a psychotic episode. Staff had been having issues with C1's increased behavior, including refusing meds for 2 days.
C1 is still at the hospital and there is no word on their return as of today.
No deficiencies were observed or cited during todays visit.
An exit interview was conducted with Melody Laflam, Caregiver, to whom a copy of this report, the Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during at the conclusion of the visit.
