Licensing Program Analyst (LPA) Tiffany Holmes conducted a Case Management visit regarding the license. LPA identified herself and discussed the purpose of the visit with Administrator Charity Bailey.
LPA toured the physical plant and dropped off the temporary license to the administrator.
No deficiencies were issued during this visit.
An exit interview was conducted with Charity Bailey to whom a copy of this report, along with Licensee/Appeal Rights (LIC 9058 03/22), were provided at the conclusion of the visit.
